Introduction: Acute Myeloid Leukemia (AML) is a heterogenous group of hematological neoplasms with morphological, cytogenetic and molecular abnormalities resulting in maturation arrest of myeloid cell lines.&nbsp;
In this report we describe our experience with Hypomethylating agents (HMA) in combination with Venetoclax (Ven) in patients not fit for intensive induction therapy in a real-world setting.
Method: We conducted a retrospective chart review of AML patients diagnosed between January 2020 and December 2021. Older adults were defined as patients greater than 60 years of age. Patients with comorbidities (renal failure, active cardiac, pulmonary and hepatic disease) active opportunistic infections and poor performance status (ECOG &gt;2) were also deemed not suitable for intensive induction therapy.
Results: Alternative induction was given with HMA (5 Azacitidine) with Ven. Patients were evaluated for treatment response after 2 cycles.
Between January 2020 and December 2021, 76 patients were diagnosed with AML. Low intensity alternative induction n= 22 The median age of patients given alternative induction was 55 years (range 26-80 years). &nbsp;Older adults (&gt;60 years of age) have increased from &lt; 10 % to 31 % (n=24) in the current cohort (Alam A et al Blood 2014; 124 (21)
The response to HMA and Ven consisted of CR 52 % (11/21) with 1 patients ineligible for evaluation (early death day + 8). The median progression free survival is 164 days (range 54-554 days).&nbsp; 5 patients have relapsed after achieving complete remission.&nbsp;
Conclusion: The combination of HMA and Ven is an effective treatment modality in newly diagnosed patients with AML who are not eligible for intensive induction. The response to treatment has improved from &lt; 10 % (HMA alone) to more than 50 % with HMA + Ven in an older cohort or those with comorbidities.

Abstract:
a-Background and purpose: In spite of approval of new therapeutic modalities, the clinical outcome of patients with &nbsp;AML remains unsatisfactory. PD-L1 plays an important role in the progression of malignant disorders so We aimed to study the association between the clinical characteristics of &nbsp;AML patients and the expression of PD-L1 &nbsp;by real-time RT-PCR.&nbsp;
b-Methodology: We conducted a prospective cohort study on patients aged more than one year old with a confirmed diagnosis of de-novo AML. All patients were subjected to routine examination and conventional induction therapy according to the center's protocol without the interference of the study's investigators. The primary outcome of this study was to estimate the association between PD-L1 expression and the clinical characteristics of the patients, Chi-square test, Mann-Whitney test and One-way ANOVA were used to determine significant relations and correlations.&nbsp;
c-Results and discussion: Overall, the median V1 and V2 expression was 0.033 (0.0001 &ndash; 2.016) and 0.067 (0.0001 - 8.05), respectively. The median v1/v2 ratio expression was 0.63 (0 - 28). The expression level of V1, V2, and V1/V2 ratio showed no significant difference among different age groups (p =0.099, 012, and 0.98, respectively). Likewise, there was no significant association between the patient's clinical presentation and V1, V2, and V1/V2 ratio expression (p= 0.27, 0.99, and 0.825, respectively). Concerning the relation between PD-L1 expression and treatment response, the V1/V2 ratio was significantly higher in patients with incomplete remission than in patients with complete remission. However, there was no statistically significant relationship among different prognostic risk groups and V1 (p= 0.118) or V2 (p =0.359).&nbsp;
d-Conclusions: PD-L1 was expressed in AML cases with variable degrees. PD-L1 v1/v2 expression levels were correlated to blast percentage. The V1/V2 ratio was significantly higher in patients who showed incomplete remission than in patients with complete remission.&nbsp;

AP03 - Association between Programmed Cell Death Ligand- 1 Expression Using Real-time RT-PCR and Clinical Characteristics of Patients with Acute Myelo

Abstract:
Trisomy 21 (T21), the most common type of +21, is found in nearly all subtypes of hematological malignancies. T21, which results in the development of Down syndrome (DS), is the most frequent numerical chromosomal aberration. DS is associated with broad spectrum of hematological findings and almost all neonates with DS have quantitative and / or qualitative disorders of myeloid compartment. Of those hematological abnormalities, transient abnormal myelopoiesis (TAM) and acute myeloid leukemia (AML) have common genetic abnormalities, i.e., T21 and acquired mutation in the key hematopoietic transcription factor GATA1, and form a continuous spectrum, referred to as myeloid proliferations associated with DS (ML-DS).
Children with DS due to an extra copy of chromosome 21 have an &sim; 150 fold increased risk of developing AML in first five years of life; most cases being that of acute megakaryoblastic leukemia (AMKL). The unique morphological, immunophenotypic, molecular genetic, and clinical characteristics that distinguish it from other forms of AMKL, including GATA1 mutations; serve as the rationale for the recognition of this form of leukemia as a distinct type in the WHO classification. ML-DS is usually preceded by a unique clonal pre-leukemic myeloproliferative disorder, TAM, in first few weeks of life; the circulating blasts of which are morphologically and phenotypically similar to the predominant form of AML in children with DS. This disorder resolves spontaneously in &lt; 3 months, but &sim; 20 &ndash; 30% progress to develop ML-DS with 1 &ndash; 3 years.&nbsp;
This presentation attempts to provide a comprehensive overview of available literature and recent advances with a focus on presenting the valuable body of knowledge available till date for better understanding of the complex molecular landscape of myeloid proliferations in DS with insights into the model of stepwise leukemogenesis.

AP04 - Myeloid Proliferations Associated with Down Syndrome

Abstract:
Background: Time consuming and cost ineffective screening, diagnostic and follow up methods of NHL (DLBCL) drive us to evaluate FT-IR as a method to overcome those pitfalls.
Methods: A case control study in which serum samples of patients of NHL (DLBCL) were examined with FT-IR, each range of FT-IR curve examined. Correlation between clinical data and FT-IR results done to detect sensitivity, specificity and precision of each range.
Results: In this study as regard accuracy, the best result seen in range six, five, three, and one 59.26%, 55.56%, 55.56%, 48.51% and 14.81% respectively. Least accuracy &nbsp;seen in range two 14.81%. As regard sensitivity, the best 2 ranges are range six and five 100% &nbsp;(both ranges has the higher accuracy also), while range two has the least sensitivity 7.14%. By depending on these 2 ranges with best sensitivity, we can use FT-IR as a potential tool for the detection, identification of Non-Hodgkin lymphoma patients, and to give an idea about those patients not in complete response &ldquo;prognostic&rdquo;. As regard specificity, range 3 was the best 92.31%, by depending on this range we can use FT-IR in detection of patients in complete response after receiving treatment &ldquo;prognostic&rdquo;.
Conclusions: FT-IR spectroscopy can be used to access serum sample of suspected NHL DLBCL patient, help in screening, rapid diagnosis, and access response to therapy.

AP05 - Fourier transform infrared spectroscopic studies for patients with Non- Hodgkin lymphoma

Abstract:
Background:Allogeneic hematopoietic cell transplantation (alloHSCT) is the only available curative therapy for hemoglobinopathies, although promising results from gene therapy are evolving. Access to transplant is limited for many patients due to lack of suitable match donors.&nbsp;
Methods:We are reporting the outcome of haploidentical transplantation for 3 patients with Thalassemia major, using T-cell replete peripheral blood stem cells followed by 2 doses of cyclophosphamide post-infusion.Calcineurin inhibitors and Mycophenolate mofetil were used as GvHD prophylaxis.All patients have received two cycles of pre-transplant immunosuppressive therapy(fludarabine and dexamethasone) were given, followed by reduced-toxicity conditioning.
Results:The first patient is a 3-year-old female (hepatomegaly, Ferritin 3600 ng/ml, and liver biopsy showed stage III fibrosis). The donor was her mother, she had moderated donor-specific antibodies. Neutrophil and platelet were engrafted on days 13 and 19, respectively, with no viral reactivation, acute or chronic skin graft vs host disease (GVHD), she has mixed donor chimerism, and she is transfusion independent for more than 3 years.The second patient is a 5-year-old female (hepatomegaly, Ferritin 2000 ng/ml, and no fibrosis in liver biopsy). The donor was her mother, she has no donor-specific antibodies. Neutrophil and platelet were engrafted on days 15 and 20, respectively. She developed CMV reactivation, BK cystitis, and grade 3 acute, she has full donor chimerism, no chronic GVHD 9 months post-transplant.The third patient is an 11-year-old female (hepatomegaly, Ferritin 1000 ng/ml, and liver biopsy showed stage I fibrosis). The donor was her mother, she has no donor-specific antibodies. Neutrophil and platelet were engrafted on days 11 and 15, respectively. She developed CMV reactivation, no GVHD, she developed secondary graft failure 90 days post-transplant.&nbsp;
Conclusion:The use of T-cell repleted haplo-HSCT with post-transplant cyclophosphamide has increased the pool of available donors and the number of transplanted thalassemia patients,considerable pretransplant evaluation is required to assess the suitability of transplant.

AP06 - Haploidentical Stem Cell Transplantation for thalassemia major Using Post-Transplant Cyclophosphamide

Abstract:
Background and Purpose Anemia is known to impact quality of life and survival in both renal and non-renal patients. End stage kidney disease (ESKD) patient&rsquo;s survival substantially improves post transplantation. Observational studies have reported better patients and graft outcomes in non anemic renal transplant recipients. Patients with post-transplant anemia (PTA) represent a distinct subset of CKD population. The aim of this extended literature review is to define the role of ESA use in improving hemoglobin (Hb) level and graft survival in patients with PTA.
Methods An extended literature review was done to identify randomized controlled trials (RCTs) with patients with PTA as the study population, the use of erythropoietin stimulating agents as the intervention, and the renal function and Hb level as the outcomes. Medline, Google scholar, Scopus, and CINHAL data bases were searched and papers meeting the pre-set inclusion criteria were identified.&nbsp;
Results and Discussion A total of 163 papers were identified. After screening the results, four papers met the inclusion criteria and were included for review. 2/4 papers recruited patients with early PTA, while 2/4 papers recruited patients with late PTA. The early PTA papers were not consistent in reporting the effect of ESA in improving renal outcomes. Both studies showed that using ESA had no additional benefit in anemia treatment. 2/4 studies looked at late PTA. The study designs were similar and the follow up period was 2-3 years. Both studies showed a better graft survival in the higher Hb group.
Conclusion In the case of early PTA, the benefit of using ESA is not clear. The two RCTs studying the effect of ESA in patients with late PTA showed that targeting higher Hb levels was associated with better graft function. The optimal Hb target and the utility of intravenous iron need further clarifications.&nbsp;

AP07 - Extended Literature Review of the role of erythropoietin stimulating agents (ESA) use in the management of post renal transplant anaemia

Abstract:
Leukemia is one of leading causes of death despite the significant improvement of survival. &nbsp;We aimed to assess the impact of absolute monocyte count (AMC) and absolute lymphocyte count (ALC) recovery on overall survival (OS) and leukemia free survival (LFS) in AML. Study design: 83 de novo AML cases presented to King Abdullah Medical City-HC, KSA were enrolled. The hemogram parameters including differential leukocyte counts were collected sequentially at days 1, 14, 21 and 28. Results: High AMC-D28 (&ge;0.8x109/L) was associated with shorter OS and LFS, P value 0.012 and 0.003 respectively. On &nbsp;multivariate models, high AMC-D28 was shown as an independent prognostic factor associated with poor OS and LFS ( HR3, P value 0.02) and (HR 5, P value 0.01) respectively. High ALC-D28 (&ge;0.35x109/L) was associated with prolonged OS and LFS survival, P value 0.032 and 0.016 respectively. On multivariate analysis, ALC-D28 failed to prove the same significance. Conclusion: Low AMC emerges as an independent predictor of better outcome. This result needed to be correlated with the myeloid derived suppressor cell in the BM. Further studies are needed to verify the prognostic value of ALC with evaluation of the lymphocyte subsets. AP08 - Absolute Monocyte Count is Superior than Absolute Lymphocyte Count at Day 28 as an Independent Prognostic Factor In Acute Myeloid Leukemia

Abstract
Background and Purpose: The ADMIRAL trial demonstrated the superiority of gilteritinib to salvage chemotherapy (SC) in patients with FLT3-mutated R/R AML. This follow-up assessed long-term survivors, transplant (HSCT) outcomes, and gilteritinib safety beyond 1 year.&nbsp;
Methodology: A data cut was performed on September 20, 2020&mdash;2 years after the primary analysis. Patients who were alive without relapse, patients who underwent HSCT, and adverse events of interest (AEIs) in Years 1 and 2 of gilteritinib therapy were evaluated. Continuous variables were assessed using descriptive statistics; categorical data were reported as frequencies and percentages. Overall survival (OS) was estimated using the Kaplan-Meier method and Greenwood formula.
Results and Discussion: As of September 20, 2020, 17% (n=63/371) of ITT patients were alive (gilteritinib, n=49; SC, n=14); 16 gilteritinib-arm patients remained on treatment. Overall, 26 of 49 gilteritinib-treated patients were alive without relapse (median follow-up: 37.1 months); 18 of these 26 patients underwent HSCT and 16 received post-HSCT gilteritinib maintenance.&nbsp; Of 371 ITT patients, 83 (22%) underwent HSCT (gilteritinib, n=64; SC, n=19). Pre-HSCT CRc rates were similar across arms (gilteritinib: 63%; SC: 58%). Forty of 64 (63%) transplanted gilteritinib-treated patients received post-HSCT gilteritinib maintenance.&nbsp; Cumulative 24-month relapse rates in gilteritinib-treated patients after pre-HSCT CR and CRc were 20% and 45%, respectively. Median post-HSCT OS (landmarked to HSCT date) was similar across arms (HR=1.076; 95% CI: 0.536, 2.160). Most common AEIs during Years 1 and 2 of gilteritinib therapy were elevated ALT/AST levels; AEI incidence declined in Year 2. Cardiac AEIs in Year 2 were nonfatal cardiorespiratory arrest (n=1) and ventricular tachycardia (n=1).&nbsp;
Conclusions: A high proportion of gilteritinib-treated R/R FLT3-mutated AML patients alive without relapse received HSCT with gilteritinib maintenance. Post-HSCT gilteritinib maintenance may explain the low post-HSCT relapse rate in the gilteritinib arm. The safety profile of gilteritinib was stable at 2 years.

AP09 - Follow-up of Patients With FLT3-Mutated R/R AML in the Phase 3 ADMIRAL Trial

Abstract:
Introduction: N8-GP is an extended half-life recombinant factor VIII for the prevention and treatment of bleeds in haemophilia A Patients. This analysis reports maintenance of long-term treatment satisfaction (TS) with N8-GP in patients of all ages with severe haemophilia A from the non-randomised, phase 3 pathfinder8 trial.
Methods: Patients were recruited from pathfinder2 and pathfinder5 trials to receive N8-GP treatment for bleeds or prophylaxis. TS was assessed with the Haemophilia Treatment Satisfaction questionnaire (Hemo-SATA) and (Hemo-SATP). &nbsp;Total scores were derived from answers to all questionnaire domains. Baseline of pathfinder2/5 (BL2/5) and pathfinder8 (BL8) scores were compared with those at the end-of-trial in pathfinder8 (EOT). Treatment duration between BL2/5 and EOT was up to 8.4 years; BL8 to EOT was up to 2.1 years.&nbsp;
Results: At EOT, (96.3%) of patients from pathfinder8 had answered at least one question in the Hemo-SAT. In adults, mean total Hemo-SATA scores decreased significantly by 5.4 (p&lt;0.001) from 15.0 at BL2/5 to EOT, and by 3.0 (p&lt;0.001) from 11.9 at BL8 to EOT. Hemo-SATA Efficacy, Burden, and Ease and Convenience domains saw the largest changes in score from BL2/5 to EOT (&ndash;10.2, &ndash;6.5 and &ndash;5.8, respectively; p&lt;0.001). Significant changes in these domains were also observed from BL8 to EOT (&ndash;4.0,&ndash;2.8 and &ndash;3.5, respectively; p&lt;0.05).For parents, mean total Hemo-SATP scores decreased significantly by 8.9 (p&lt;0.001) from 17.6 at BL2/5 to EOT, and by 1.2 (non-significant) from 11.1 at BL8 to EOT. Hemo-SATP Ease and Convenience, Efficacy and Burden domains saw the largest changes in score from BL2/5 to EOT (&ndash;12.6, &ndash;11.3 and &ndash;11.2, respectively; p&lt;0.001).&nbsp;
Discussion/Conclusion: Long-term improvements in treatment satisfaction, or maintenance of treatment satisfaction benefits, were observed with N8-GP in patients with severe haemophilia A of all ages. Efficacy, Burden and Ease and Convenience domains showed the largest benefits.

Abstract
Background and Purpose:Sickle cell disease is an inherited autosomal recessive,caused by a single gene mutation in beta chain of normal adult hemoglobin,leading to abnormal sickled hemoglobin.Hemoglobin D-Punjab is formed by a missense mutation at the 121st position of beta chain.Although HbD trait and Homozygous D are asymptomatic,in compound heterozygous HbSD disease,HbD Punjab interacts with HbS to enhance polymerization and sickling process,and produce moderately severe sickle cell disease phenotype. The aim of the project is to establish the clinical&amp;laboratory features of patients with SD hemoglobin admitted at Sultan Qaboos University Hospital and the impact of hemoglobin F and other modulators on the disease severity.&nbsp;
Methodology:a retrospective cohort study was conducted among 50patients with SCD SD Punjab treated at SQUH.Patient's related data were accessed from Electronic Patient Records at the Hospital Information System and analyzed using program of Statistical Package for the Social Sciences. Results:Out of 50 patients with SD-Punjab,60%were males and40% females.It had been found that most of the patients have A+blood group and no A-negative,B-negative,AB-negative groups had been recorded.The mean hemoglobin of the group was9.88g/dl which is relatively low.77.4% of patients presented with vaso-occulsive crises of less than3 and the frequency of strokes was13.2%.The clinical parameters that were present are:splenic sequestration with34%cases of splenectomy,gallstones,avascular necrosis,and no cases of priapism.Additionally,it had been observed that as HbF increases,the level of HbS decreases.Finally,mortality rate was7.5%which is relatively high.However,it was found that hydroxyurea lowers the progression of the disease as no death cases had been reported among patients who are on hydroxyurea treatment (58% of all cases).
Conclusion:SCD SD Punjab is the 3rd most common genotype of SCD in Oman,and associated with many serious complications.Most frequent complications include:recurrent VOC, ACS, AVN.No cases of priapism were reported in males of SD group.Hydroxyurea is associated with improved prognosis and better survival in this cohort of patients.

AP11 - Clinical and Laboratory Features of Sickle Cell Disease S/D Punjab in Oman - Retrospective Cohort Study

Abstract:
Background and purpose: Hemosiderin precipitation in endocrine tissues of patient with TDT leads to progressive glands dysfunction. In this study we aimed to assess the adrenal gland reserve of a cohort of thalassemic children who receive frequent blood transfusion, defined as more than 10 packed red blood cells (PRBC) during their life. Moreover, we aimed to determine the prevalence of adrenal dysfunction (primary or secondary) and therefore to intervene timely to avoid any adverse outcomes from undiagnosed adrenal dysfunction.&nbsp;
Methodology: A cross sectional study on 90 patients with TDT. They were evaluated clinically for symptoms and signs of adrenal insufficiency. They had basic laboratory workup including CBC, ferritin level, basal cortisol, and ACTH levels followed by low dose Synacthen test (LDSST) which helps identifying patients with secondary or partial adrenal insufficiency.
Results and discussion: Ninety patients were recruited and 76 (38M: 38F) of them have completed the study. Their mean age 9.30&plusmn;4.90 years. The prevalence of adrenal insufficiency was 13.2%, secondary in nature (peak cortisol below 350nmol/L). In this study, there was no correlation between mean serum ferritin and development of adrenal insufficiency. This goes with a previously reported studies that concluded ferritin is not a very sensitive tool to rely on, as no direct relationship was found between ferritin level and other endocrine disorders. Majority of the studied patients, 57.9 % had inadequate response to LDSST (peak cortisol between 350-550nmol/L), indicating partial adrenal insufficiency, for which they will require steroid cover only during sick days.&nbsp;
Conclusion: Transfusion dependent thalassemic patients with deficient adrenal reserve can be missed during intercurrent infection or illness and may develop adrenal crisis. We recommend assessing adrenocortical functions routinely for children with TDT and implement a prompt replacement therapy plan to avoid life threatening effects.

AP12 - Adrenal dysfunction in Omani Children with Transfusion Dependent Beta-Thalassemia (TDT): Does it deserve a mandatory routine assessment?

Abstract:
Background/Aims: N8-GP is a recombinant, glycoPEGylated, extended half-life factor VIII (FVIII) replacement product approved for the treatment of patients with haemophilia A. This clinical trial evaluates safety and efficacy of N8-GP in previously untreated patients (PUPs) with severe haemophilia A.
Methods and materials: Pathfinder6 (NCT02137850) is an ongoing, open-label, single-arm, phase 3a trial investigating N8-GP treatment in severe haemophilia A PUPs, enrolling male children, &lt;6 years old, with no exposure to FVIII concentrates and no FVIII inhibitors. The primary endpoint was the FVIII inhibitor incidence (two consecutive tests &ge;0.6 Bethesda units (BU); high-titre was &ge;5 BU). For this analysis, &ge;50 patients had &ge;50 exposure days (EDs) to N8-GP. Patients with &ge;10 EDs or inhibitors were included in inhibitor incidence analysis. Patients &lt;24 months old were treated with pre-prophylaxis or prophylaxis. Patients &ge;24 months old were treated with prophylaxis.
Results: Eighty patients received &ge;1 N8-GP dose and were included in this analysis. Of the 67 patients eligible for FVIII inhibitor analysis, 20 developed inhibitors (10 high-titre), resulting in an incidence rate of 29.9% (14.9% high-titre). Seven patients began immune tolerance induction therapy; 5 were successful, 1 withdrew and 1 has ongoing therapy. Sixty-five patients received N8-GP prophylaxis for an average 2.17 years. In these patients, the median ABR (interquartile range) was 1.42 (0.76; 3.13), 91.3% of bleeds were treated with &le;2 injections and the haemostatic success rate was 90.5% (Table). No unexpected AEs were observed. Temporarily decreased incremental recovery was observed in 17 non-inhibitor patients (examined in a separate abstract).
Conclusions: Compared with previous studies, patients in pathfinder6 had an inhibitor incidence within the expected range and a high-titre incidence within the lower range. Patients on prophylaxis had a median 1.42 ABR and a 90.5% success rate for treatment of bleeding episodes, indicating that PUPs can be successfully treated with N8-GP.

Abstract:
Background and purpose: Acute splenic sequestration crisis (ASSC) is considered as one of the earliest life-threatening complications seen in patients with sickle cell disease (SCD). In this retrospective study we aimed to assess the possible correlation between SCD genotypes and the phenotype of recurrent ASSC.
Methodology: 140 Omani children attending Sultan Qaboos University Hospital (SQUH) during the period between 2009-2020 were recruited for the study after taking informed consent/assent. Specific risk factors that might contribute to recurrent ASSC and the role of use of hydroxyurea (HU) in preventing the ASSC in these patients were also studied.&nbsp;
Results and discussion: Among the 140 children who were involved in this study, there were 83 boys (60%) and 63 girls (40%). &nbsp;The majority of study patients 67% (n. 94) were Sickle/B thalassemia S/B (57 were S/B+ and 37 were S/B0). Thirty percent of patients (n. 42) were S/S, and only 3% (n. 4) were S/D. About 57% of the ASSC patients had documented fever (T&gt;38 0C) and 73% had low MCV (&lt;70 pg/l). Seventy nine patients received HU, and 39 of them underwent spleenectomy. Hydroxyurea therapy prevented the need for splenectomy in 4 patients.&nbsp;
Conclusion: In conclusion, this study suggests a positive correlation of sickle beta-thalassemia SCD genotype with the increased frequency of ASSC. Hydroxyurea role in reducing or preventing ASSC remains questionable. A Prospective multi-centred study is required.&nbsp;

AP14 - Genotype Phenotype Correlation of Recurrent Splenic Sequestration in Omani Children with Sickle Cell Disease

Abstract:
Thalassemia Centre serves patients with &beta;-Thalassemia major and other hemoglobinopathies, those patients develop co-morbidities linked to the underlying disease and complications of chronic transfusions. Some patients are &ldquo;immunocompromised,&rdquo; especially those following up in the center and had undergone a Bone Marrow Transplant.&nbsp; Quality and risk management tools for many are merely scientific theories and are not helpful during crisis where rapid decisions are needed. In contracts with this point of view, the leadership at the center had chosen to benefit from these tools and utilize them efficiently.&nbsp; This abstract highlight Thalassemia Centre experience of applying the available quality and risk management tools to ensure effective response to the pandemic.
&nbsp;METHODOLOGY Thalassemia Centre applied various quality and risk management tools to respond to COVID-19 pandemic, these tools included &lsquo;Risk registers, Failure Mode and Effect Analysis, Business Impact Analysis in addition to testing different infection emergency scenarios such as influx of infectious patients to the center. These tools were helpful to proactively predict expected failures and create preventive action plans accordingly.&nbsp;
RESULTS AND DISCUSSION The quality and risk management tools helped the Centre to rearrange the service workflow and provide a systematic approach to change. Preventive action plans included process modifications related to the patients and the facility. Daily checks of patient infectious status for future appointments, cancellation of non-urgent clinics, introduction of Teleconsultation clinics, home medication delivery, modification of blood requirement calculations were some of these strategies.&nbsp;
CONCLUSIONS The quality improvement and risk management tools were not only concepts and theories, but rather, assisted Thalassemia Centre to improve workflow, strengthen the system, and enhance learning to sustain improvement. The pandemic had low impact on the services and processes at Thalassemia Centre with zero business interruption days since start of the pandemic .

AP15 - Emergency preparedness and business continuity at Dubai Thalassemia center during covid-19 pandemic – using quality improvement and risk mana

Abstract:
Introduction: Hemolytic anemias can be classified as primary or secondary. AIHA related to autoimmune disease, infections, drugs, and hematological malignancies. The association between AIHA and non-Hodgkin's lymphoma (NHL) is well known (10%) and few cases series reported the outcomes of such cohort. We described a case of AIHA related to NHL in remission and concomitant hereditary G6PD disease.
Case description: A 46-year-old male patient in remission of NHL treated 30 years ago with 6 cycles of chemotherapy. He was diagnosed with autoimmune hemolytic anemia in 2018 and treated with rituximab and steroids. He presented with 4 days history of dyspnea, fatigue, jaundice, and dark urine. He was using new herbal medicine for weight reduction. He was febrile 38.5, tachypneic 28, tachycardiac 123, blood pressure was 103/72 mmHg and saturating 98% on 2L oxygen. &nbsp; Clinical examination revealed jaundice along with hepatosplenomegaly. Laboratory investigations were significant for acute kidney injury (Cr 174 micromol/L), and acute hemolytic anemia (Hgb of 56 g/L, with low Hct 0.14 L/L, MCV 92.6 fL, RDW 18.2%, Retic Count 4.2%, haptoglobin &lt;0.1 g/L, unconjugated bilirubin of 30.2 micromol/L, LDH 885 IU/L). Coombs test was positive for IgG 4+ and negative for C3d and RBC Agglutinin on blood film. The G6PD level was low (G6PD &lt;.28 units/g) and he denied family history or prior diagnosis of G6PD. The diagnosis was hemolytic anemia due to concomitant G6PD hemolysis and AIHA/ NHL. &nbsp;He was managed with IV hydration, Methylprednisolone, folic Acid and PRBC transfusion.
Discussion and Conclusion: In AIHA/NHL, it is fundamental to screen for other causes of hemolytic anemia. As in our case, he had AIHA / NHL with frequent hemolysis episodes over 4 years, that required prolonged courses of steroid and rituximab. We diagnosed him with G6PD deficiency which can result in an overlap between AIHA and G6PD Hemolysis.

AP16 - Autoimmune Hemolytic Anemia (AIHA) and G6PD Hemolysis in Patient with Non-Hodgkin’s Lymphoma (NHL)

Abstract:
Background and purpose START (NCT03591575) evaluated the safety and efficacy of the oral iron chelator deferiprone (DFP) in children with transfusion-dependent beta-thalassaemia who did not yet meet criteria for starting chelation therapy per standard practice.&nbsp;
Methodology Infants and children receiving regular blood transfusions (&ge;2) with serum ferritin (SF) levels of 200&ndash;600 &mu;g/L were randomly assigned 1:1 to receive oral DFP or placebo, until SF levels exceeded 1000 &mu;g/L at two consecutive visits or, if the threshold was not reached, for 12 months. DFP was initiated at 25 mg/kg/day and increased to 50 mg/kg/day (or to 75 mg/kg/day, based on iron load). The primary endpoint was the proportion of patients in each group with SF &lt;1000 &mu;g/L.&nbsp;
Results and discussion Overall, 32 patients were enrolled per group. Mean age (standard deviation) was 3.03 (2.42) and 2.63 (1.70) years in the DFP and placebo groups, respectively; there were no significant between-group differences in baseline characteristics. The primary reason for withdrawal was SF exceeding 1000 &mu;g/L at two consecutive visits, occurring in 25% and 63% of patients receiving DFP or placebo (P=0.0051), respectively. At 12 months, 65.6% of DFP-treated patients had SF levels &lt;1000 &mu;g/L versus 37.5% of placebo-treated patients (P=0.0446), with a more rapid SF increase in the placebo group (P=0.0407). There were no significant between-arm differences in the number of adverse events (AEs; P=1.0000), serious AEs (P=0.4258) or AEs possibly related to DFP (P=1.0000). Two DFP-treated patients withdrew owing to agranulocytosis or neutropenia that resolved after withdrawal.
Conclusions Initiation of DFP chelation therapy at a lower SF value than currently recommended was effective in preventing iron overload in most participants. The safety and tolerability profile of DFP administered for up to 12 months was similar to that established in older age groups and there were no instances of iron depletion.

AP17 - Safety and efficacy of early-start deferiprone in infants and young children with beta-thalassaemia (START study)

Abstract:
Background and purpose FIRST (NCT02041299) was a 12-month, phase 4, randomized, open-label study in adults and children with sickle cell disease (SCD) or other anaemias requiring treatment for transfusional iron overload. Deferiprone (DFP) was non-inferior to deferoxamine (DFO) for change in liver iron concentration (LIC) from baseline to month 12 (primary efficacy endpoint), and had an acceptable safety profile. Here, we report a subgroup analysis of paediatric participants in FIRST.
Methodology The subgroup analysis included children (2&ndash;16 years old) who received either oral DFP three times daily or subcutaneous DFO infusion 5&ndash;7 days a week. Statistical significance between DFP- and DFO-treated groups was calculated via t-test for continuous variables and Fisher&rsquo;s exact test for discrete variables.
Results and discussion The safety population included 86 DFP-treated and 42 DFO-treated children; most (75.6% and 80.9%, respectively) had a primary diagnosis of SCD (HbS). Mean (standard deviation [SD]) age was 9.9 (3.7) and 10.9 (3.0) years for DFP and DFO, respectively (P=0.09). Five children (all DFP-treated) withdrew owing to adverse events (AEs), and 19 (DFP, n=14; DFO, n=5) for other reasons. There was no significant difference in the incidence of AEs (P=0.77; including neutropenia [P=0.30]), severe AEs (P=0.10), serious AEs (P=0.16) or withdrawals due to AEs (P=0.17). DFP-treated children had a higher incidence than DFO-treated children of non-serious AEs at least possibly related to treatment (P=0.01) and of elevated liver enzymes (P=0.03). One DFP-treated child developed agranulocytosis during a parvovirus infection, which resolved the following day. There was no significant difference (P=0.57) in mean (SD) LIC change from baseline to 12 months with DFP (&minus;3.39&plusmn;4.24 mg/g) versus DFO (&minus;2.99&plusmn;3.16 mg/g).
Conclusions Treatment with DFP was similar to that with DFO in reducing LIC in children. No new safety concerns were observed that have not been previously noted in other populations.

AP18 - Randomized controlled trial of the efficacy and safety of deferiprone: subgroup analysis of iron-overloaded paediatric patients with sickle cel

Abstract:
Background and Objectives Sickle cell anemia (SCA) is one of the most prevalent autosomal recessive genetic disorders worldwide, Similarly, in Oman, the disease is very prevalent too. Although siblings share the same parental beta-globin genes and same mutation, but there is variation in the behaviour of SCA even among siblings. The study aims to identify the clinical and laboratory variables that impact the variation in behaviour among siblings with SCA.&nbsp;
Methodology&nbsp; We performed a retrospective analysis to look at the clinical and laboratory variables of siblings with SCA at Sultan Qaboos University Hospital (SQUH). Accordingly, we studied 85 families who are known to have more than one sibling with SCA. We looked at clinical features including Number of vaso-occlusive crisis, acute chest syndrome, stroke, acute hepatic and splenic sequestration and avascular necrosis of hips. For the labs, we analyzed their baseline Hb, MCV, Hb F, HbS, WBC, platelets, bilirubin and LDH. &nbsp;A Chi-square test had been used to test the consistency of clinical features, whereas an intra-class correlation coefficient test was used to assess the variability of laboratory features.&nbsp;
Results and discussion&nbsp; Our results of indicate significant variability correlation between siblings, with correlation coefficient (r) of MCV was (r = 0.422; P = .007), Hb was (r = 0.309; P=.049), LDH was (r = 0.368; P = .023), HbS was (r = 0.368; P=.014). since a correlation coefficient is less than (0.5), this indicates that there are low consistency in these laboratory variables among siblings. Whereas, in the clinical features, using the Chi-square test showed no significant variability among siblings of SCA.
Conclusion Our findings suggested existing of variability in laboratory parameters among siblings with SCA, although they share similar parental beta-globin genes, and further work is needed to identify the environmental and genetic factors that impact these variables.
