Dr. Heather Silver is a primary care internal medicine physician at The Emory Clinic. She provides direct patient care and is involved in student and internal medicine resident education at The Emory Clinic and Grady Hospital in Atlanta.
Starting in February 2021, clinicians in a primary care clinic began using handouts, an AHA instructional video, and home blood pressure devices during routine clinic visits to teach patients with hypertension how to measure their blood pressure at home. Incorporating Self-Measured Blood Pressure (SMBP) into patient care can improve blood pressure control. The goal of implementing this strategy to teach SMBP is to Improve hypertension control by 5% .

Disrupting the cycle of health inequities requires a comprehensive and collective approach that mobilizes the community to lead and be a part of the solution. Research has shown that to effectively reach the community and to influence healthy behavior change, a bottom-up, community-centered approach, that provides culturally credible information from trusted community partners is paramount. As a result, a diverse group of stakeholders with a common goal to achieve health equity on the Westside, the Westside Health Collaborative, partnered with Morehouse School of Medicine and two neighborhood barbershops to implement a health promotion model that meets the community where they are. This pilot focuses on building the capacity of community barbers to identify and address mental health needs of clients who frequent the barbershop, as well as offer recurring blood pressure screenings and medical home linkage. The model also includes a designated Community Health Navigator to provide support with linkage to care and follow-up, while also supporting individuals with wraparound services. This presentation will highlight how this unique model aims to improve health outcomes for a population that most commonly faces both stigma and barriers to access through non-traditional settings.

JumpStart is an eight-week community health initiative and wellness program designed to improve cardiovascular health&mdash;and more specifically, reducing blood pressure. JumpStart is free and open to anyone with high blood pressure, and the program combines exercise, nutrition, and clinical education in a weekly two-hour class format. The program was developed and piloted by Good Samaritan Health and Wellness and utilizes community partners including New Beginning Therapeutic Services (exercise and health coaching), University of Georgia Extension Service (nutrition education), and Reinhardt School of Nursing &amp; Health Sciences (hands-on clinical education for nursing students). Good Samaritan provides clinical oversight and program staff as well as clinical education, program development, and program evaluation. In its inaugural pilot year, approximately 75 individuals will complete the eight-week program and many experienced one or more lifestyle improvements including reduction in blood pressure, weight, heart rate and/or improved eating, exercise, and smoking cessation habits. In just eight weeks, approximately 70% of participants experienced a reduction in systolic blood pressure and had diastolic blood pressure that fell within the normal range. In addition, 90% of participants reduced their resting heart rate and 68% lost weight.

A Professional Organization's Role in Health Equity

In conducting a Community Health Needs Assessment, Floyd Medical Center determined that disparities of care existed in the minority community in the area of heart care. These barriers especially impacted the African American Community. With a mission to improve health, elevate hope and advance healing for all, Floyd leaders became determined to address those challenges. That determination led to the development of the More Heart community engagement initiative. This effort began with the formation of the More Heart Advisory Board, made up of business, civic and religious leaders in northwest Georgia&rsquo;s minority community. This board was tasked with helping guide Floyd&rsquo;s efforts to improve heart health in those communities. Dan Bevels, Aimee Griffin, and Tashia Twyman will outline the early success of this initiative, despite the challenges brought on by the pandemic.

Addressing Health Care Disparities with More Heart

This presentation will provide information about evidence-based PSE changes implemented to prevent and control hypertension. Theses implemented changes have led control rate of 63% in total number of patients who have achieved control blood pressure control less than or equal to 140/90 (&lt;= 140/90) within South Health District 10 county coverage area.

Preventing and Controlling Hypertension Through PSE Changes 

Since 2006, the South Central Health District has led diabetes support groups for diabetics and their families living in the ten county health district. Our goal was to eventually obtain accreditation through ADA or AADE so that formal diabetes education could be provided in our area. In 2015, a partnership with Meadows Regional Hospital allowed the South Central Health District to provide DSMES for diabetics in our area and those receiving services at Meadows Regional Hospital. In 2020, the Georgia Department of Public Health began work on a state-wide accreditation which will allow us to workmore independently on DSMES in our health district.

Diabetes Self-Management Education Initiative to Local Public Health Working Under Umbrella of State DPH Accreditation

Rural residents are less likely to engage in leisure-time physical activity (PA) or meet PA recommendations compared to urban dwellers, especially in the South. Intersections of rurality, socioeconomics, and built environments can compound PA disparities. We examined the relationships among leisure-time PA, socio-demographics, and perceived access to recreation environments (i.e., areas around your home, indoor and outdoor exercise areas, town center connectivity, school and church facilities) in rural Georgia. This study uses baseline data from an evaluation of Healthcare Georgia Foundation&rsquo;s. The Two Georgias Initiative which funds coalitions across 11 rural Georgia counties to address health equity using locally appropriate strategies. Residents in three coalition counties (n=728) completed a mail survey in 2019. More than half of respondents (53.5%) engaged in leisure-time PA in the past month. Leisure-time PA was more common among those who were younger, male, White, not obese, lived in more rural areas, had higher levels of income and education, and worked full-time. The overall recreation environment and town center connectedness were positively associated with leisuretime PA, although differences were small for both. We will discuss how the coalitions are addressing PA disparities and offer suggestions for future interventions aimed at promoting physical activity and reducing inequities.

Built Environments and Leisure-Time Physical Activity: Exploring Inequities within Rural Communities

The purpose of this presentation is to address the landscape of telehealth services and social determinants of health for many adults living with high blood pressure. The pandemic accelerated clinician&rsquo;s plans to invest in digital health products and services. There is a need for healthcare systems, practices and providers to enhance care, during and beyond a pandemic, to minimize the impact of patient surge on facilities, reduce staff exposure to ill persons, improve access to care, overcome diagnostic uncertainty and nonadherence to prescribed treatment (Boonyasai, Rakotz et.al, 2016). 30bp is a hypertension platform to increase provider and staff engagement in implementing evidence-based practices including, advanced self-measurement blood pressure (SMBP) technology, and to increase controlled hypertension.

Community Collaboration to Address Social Determinants of Health 

The Georgia Department of Public Health&rsquo;s Chronic Disease Prevention Section (CDPS) is seeking to train health systems and community organizations on the Georgia Healthy Heart Ambassador (HHA) Blood Pressure Self-Monitoring (BPSM) program, as approved by CDC&rsquo;s Division for Heart Disease and Stroke Prevention. This training will facilitate the establishment of referral systems to evidence-based programming that supports individuals with hypertension and high blood cholesterol. DPH will offer the training as a strategy to improve blood pressure rates in Georgia by providing organizations with tools to raise awareness of high blood pressure and increase access to an affordable, and culturally competent hypertension control program in high burden communities.

Healthy Heart Ambassador Blood Pressure Self Monitoring Program

Diabetes Prevention is a pivotal part of combatting the burden of diabetes in Georgia, by preventing the onset of diabetes. The National Diabetes Prevention Lifestyle Change Program is an evidence-based program to help participants reduce their risk of developing diabetes. This presentation will walk you through standing up a National Diabetes Prevention Program and opportunities to share ideas with other organizations offering diabetes prevention through the state.
