Veterans accout for six percent of the US population and their healthcare delivery differs from the general population leaving their poor oral health and negative health outcomes a public health concern. Recent research shows that one-third of Veterans have an urgent dental need, besides being at increased risk of numerous physical and mental health conditions that have oral health complications. Since most Veterans are ineligible for dental care through Veterans Affairs (VA), organizations across the county have hosted targeted outreach events to reach these patients and address the gap and needs in dental healthcare among this population. This poster will cover in-person outreach to provide new patient exams and prphylactic care for Veterans through a no-cost event at a dental school clinic, and a preliminary needs assessment to gain insight into the challenges Veteran members face in accessing dental care.

Syndemics, or synergistic epidemics, are the aggregation of two or more concurrent epidemics in a population- which usually result in interacting public health crises. Recently, a new framework from the Rutgers Center of Alcohol and Substance Use Studies outlines the interaction between coronavirus, substance use disorders or &ldquo;opioids&rdquo;, and racism as a systemic public health crisis. Titled the &ldquo;COR&rdquo; Syndemic- these three different epidemics are correlated with increased stress on populations, especially vulnerable groups. Veteran populations in the United States are a high risk group for adverse health impacts due to the COR Syndemic. During the COVID-19 pandemic especially, Veteran populations have been under additional stress and isolation. Veteran populations have increased risk factors for COVID-19 compared to the general population, Veteran overdose mortality has increased since 2017, and Military Veterans of color are at risk of developing adverse psychological outcomes at higher rates than their White counterparts. The latest 2021 data regarding how Veterans are impacted by the novel coronavirus, opioids/substance use disorders, and racism highlight the need for increased evidence-based intervention and harm reduction practices. Oral health is adversely impacted from each epidemic within the COR Syndemic. Those with poor oral health are at higher risk of adverse outcomes from COVID-19, emergency visits due to dental pain have increased since 2018 and are often mistreated with pain management, and structural racism continues to create barriers to oral disease prevention for Black, Indigenous, and People of Color (BIPOC) at the population level. In order to address oral disease in Veteran populations, the COR Syndemic must also be understood and harm reduction strategies implemented.

Objectives: To determine whether various types of fluorides (office applied 5% NaF varnish, office applied gel/rinse [2% NaF gel/rinse or 1.23% APF gel], prescription 1.1% NaF paste or gel) versus no fluoride is more effective at preventing new caries-related restorations/extractions in Veterans at high risk for dental caries. &nbsp;&nbsp;
Methods: This retrospective analysis of longitudinal data from 10/01/2008-06/30/2018 used data from the Department of Veterans Affairs electronic dental, medical, and pharmacy records. To be included, Veterans had 3+ years of data available; met the criteria of high caries risk (received 2+ caries-related restorations within a 365-day period, [index year]); received no caries-related dental treatment or fluoride intervention in the year prior to the index year; and had a 1-year follow-up after the index year. The outcome measured was teeth with caries-related restorative treatment/extractions. Co-variates included age, gender, race/ethnicity, medical comorbidities, prescription medications, dental prophylaxis appointments, number of restorations and days between first and last restoration during the index year. Logistic regression modelled the outcome with the intervention and all covariates.&nbsp;
Results: The study sample included 68,757 Veterans; mean age=59.2&plusmn;13.5 years; 91.5%=male; 73.6%=White/Caucasian, 22.7%=Black/African American and 5.8%=Hispanic. &nbsp;Participants had a mean of 3.9&plusmn;2.6 caries-related restorations during the index year (enrollment into study) and 0.87&plusmn;1.82 caries-related restorations/extractions during the outcome year. &nbsp;Veterans receiving a fluoride varnish treatment (OR=0.71, 95%CI=0.67,0.75) or a fluoride gel/rinse (OR=0.72, 95%CI=0.67,0.76) versus no fluoride were less likely to receive caries-related treatment during the follow up period, and dose-response was observed (gel/rinse: 1vs0 OR=0.87, 95%CI=0.83,0.91and 2+vs0 OR=0.71, 95%CI=0.67,0.75; varnish: 1vs0 OR=0.84, 95%CI=0.81,0.88 and 2+vs0 OR=0.73, 95%CI=0.69,0.77). &nbsp;Receipt of prescription 1.1% NaF paste/gel versus no fluoride was not statistically significant (OR=1.03, 95%CI=0.96,1.10).&nbsp;
Conclusions: Receipt of a fluoride varnish or gel/rinse decreased the odds of future caries-related treatment. A dose-response relationship was observed, with 2+ treatments being more effective than one treatment.

Topical Fluoride Effectiveness in High Caries Risk Adults

Increasing access to care and improving both oral health and overall health require an oral health workforce working in concert. Increasing access to care and stimulating oral health innovation to include rural and underserved areas will be more successful if diverse intra- and interprofessional care teams are deployed. Regulatory and scope of practice requirements that govern all health care team members should be evaluated to ensure that access to high-quality care is a possibility for all veterans regardless of geography, level of need, or disability. Alongside traditional clinical care teams, new oral health team members such as physical therapists, community health workers, behavioral health specialists, and health informaticians can innovate care delivery for veterans. The VA has promulgated a keen awareness in innovating health workforce solutions, encouraging opportunities for oral health to be considered and elevated within an existing whole-person framework. Congress will need to ensure that VA oral health care teams have the resources and staffing to drive integrated approaches to care and achieve whole-body well-being. A focus on a diverse, efficient workforce will streamline the delivery system to meet veterans where they are, while enriching the communities they call home.

For the nearly 19 million veterans and approximately 2 million active-duty members, a positive connection to peers and a sense of duty to their country is typically central to military service. Service personnel may have experienced traumatic events during and outside of deployment. While research indicates that both Adverse Childhood Experiences (ACEs) and trauma that occurs in adulthood are unfortunately extremely common, the impact on individuals, families and communities can be addressed to mitigate potential cumulative effects on health outcomes. As one of the &ldquo;Strategic Recommendations for Improving Quality of Care&rdquo; outlined by CareQuest and the AIDPH, prioritizing trauma-informed approaches in oral health care can ultimately support veterans on their path to wellness. &nbsp;True TIC transformation can also improve clinical workflows, patient interactions, and staff engagement. TIC approaches in the health care setting are aligned with the full integration of patient-centered, team-based, and can lead to value-based care. This presentation and discussion will provide participants with foundational knowledge about trauma and the potential manifestations in clinical care settings. Practical tips for oral health professionals to consider, with use of universal trauma precautions, will be provided.

Prioritizing Trauma-informed Approaches to Clinical Care for Veterans (concurrent session)

Many veterans who are low-income, unemployed, and struggling with chronic disease or unmet health needs often experience inadequate access to dental care. Episodic and urgent dental care may not be sufficient in alleviating poor oral health to establish overall health outcomes. Recent American Dental Association Humanitarian Award Winner Theresa Cheng will describe her approach to filling gaps for veteran dental care as part of her work with Everyone for Veterans. Speakers will share strategies and opportunities for dental clinicians to meet the oral health care needs of veterans beyond an urgent needs model by generating a whole-person approach.

Expanding Veteran Oral Health Care Delivery Beyond an Urgent Needs Model (concurrent session)

Veterans cannot be healthy without good oral health. Veterans experience poorer oral health outcomes than nonveterans. These disparate health outcomes are exacerbated by a historically fragmented dental care system. Clear evidence underscores a need to improve the US oral health infrastructure to support all patients, including veterans. Understanding how veterans perceive their own oral health and how they access oral health care will be critical to implementing systemic changes that improve oral health in this population. This panel of veterans will highlight patient voices to share how their lived experience has shaped their perception and access to dental care.
